Do not use these forms if:
- You have a mandatory e-pay requirement
- You are submitting payments through the Third-Party Payments application.
The following forms allow FTB to properly apply payment from one check or money order to multiple taxpayers in an efficient manner.

Payments for individual taxpayers must include FTB 5007 or FTB 5008 for Estates and Trusts to identify the payment amount for each taxpayer.

Payments for multiple taxpayers must include FTB 5013 PC to identify the payment amount for each entity and if applicable, be accompanied by the FTB 5012 PC for new filers.

- FTB 5012 PC, Multiple Payment Submission — Business, New Filer
Note: FTB 5012 is used for each business that is marked with a Y on FTB 5013 PC. A completed FTB 5013 PC must be submitted with the completed FTB 5012 PC.
Do not co-mingle existing filers and new filers on the same form. Use FTB 5012 form for new filers. Use FTB 5013 form for existing filers.
